Caring for Cancer Survivors in the Community

Cancer Survivorship encompasses your entire journey, from diagnosis to finishing your cancer treatments. At NCCS, our mission for cancer survivors is to help our cancer survivors live with, through and beyond cancer. When our cancer survivors* stay cancer-free after completing their treatment, we work closely with our community partners to transit them back to the community. 

Delivering on Target (DOT) 
Delivering on Target (DOT) is a SingHealth initiative that aims to match patients to a compatible General Practitioner (GP) for affordable long-term primary and survivorship care. We aim to transit our Breast Cancer Survivors who are in remission seamlessly to a suitable GP. Our primary care partners have undergone training to provide cancer survivorship care. 

Breast Cancer Survivors 
Subsidised annual mammogram will be done under Screen for Life (National Health Screening Programme). In the event of abnormal mammogram results or if the survivor has any signs and/or symptoms or concerns, they will be fast tracked back to NCCS/SGH for further assessment. 

Benefits of DOT Programme
  • Consultation charge capped between $25 and $40 
  • Convenience of medication courier service from NCCS/SGH 
  • Medication at hospital subsidised rate
